Kids Drone Zone (KDZ) Please read carefully: RELEASE FORM FOR MINORS (under the age of 18) PARENTAL CONSENT REQUIRED By Signing this electronic document, being the parent or legal guardian of Minor volunteering (the “Minor”) hereby consent to and authorize the Minor to participate in Kids Drone Zone activities. I acknowledge and agree that activities are strictly on a voluntary basis, without any pay, compensation, or benefits. I agree and understand that the Minor must comply with the rules and regulations established from time to time by Kids Drone Zone and that failure to do so may result in the Minor’s immediate removal from the activity and parent/Legal Guardian will be contacted. I am aware of the nature of the activities to be performed by the Minor and recognize that in participating, there is a risk of harm or injury exists that may exist. I agree that all activities are to be performed by the Minor at the Minor’s risk and I assume full responsibility therefore. Photographic Release. I grant and convey unto KDZ all right, title, and interest in any and all photographic images and video or audio recordings made by KDZ during me or my families participation in KDZ activities, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. On behalf of myself, the Minor, and our respective heirs and personal representatives, I agree not to hold or attempt to hold Kids Drone Zone (KDZ), their population served, volunteers, or staff responsible for any injury or damage sustained or incurred by the Minor, arising out of or in any way connected with the Minor’s activities as a volunteer for Kids Drone Zone. I hereby release and discharge Kids Drone Zone, their employees, and their volunteers from any and all claims, demands, causes of action of any nature or cause, for any such injury or damage incurred or suffered by the Minor. Please read carefully I have read and understand the statements below regarding Kids Drone Zone policies and procedures. Statement of Authorization: In the case that your child becomes ill during the program, you will be contacted as soon as possible. If the parent or guardian is unable to be reached, the child’s emergency contact will be notified. It is the responsibility of the parents or guardians to arrange for the child to be picked up from the center as soon as possible. .In the case that your camper or anyone in the immediate household of the camper develops a reportable communicable disease as defined by the State Board of Health, it is the responsibility of the parent to notify the KIDS DRONE ZONE within 24 hours or the next business day in order for the KIDS DRONE ZONE to take proper action, except in the case of life-threatening diseases which must be reported immediately. My signature authorizes the management and staff of the KIDS DRONE ZONE to act for me according to their best judgment in the event of a medical emergency and/or routine medical care. I/we grant permission for emergency medical treatment and/or routine medical care by the KIDS DRONE ZONE camp staff, a rescue squad, or private physician and/or hospital or emergency health care facility staff, under the same circumstances as above, if needed. Any such action will be taken in the best interest of my child and will be reported to me/us as soon as possible. My signature waives and/or releases KIDS DRONE ZONE. I release Kids Drone Zone from any and all liability and/or financial responsibility for any medical expenses incurred. The following information is important for the safety and protection of your child. Please read this information and sign below. • I understand that my child will not be released to any person(s) not listed on the enrollment form. • I understand that my child will not be released to any person(s) who seems to be under the influence of drugs or alcohol. • I understand that I am not to leave my child at the Kids Drone Zone or program site unless a KIDS DRONE ZONE Camp staff member or volunteer is there to receive and supervise my child. • I understand that it is my responsibility to sign my child in the morning and sign my child out before leaving in the afternoon. Sign-in/Sign-out sheets are available as you arrive at the program area. • I understand that my child will not be allowed to leave the program with an unauthorized person. Any person authorized to pick up my child must be listed on this form. Authorization by telephone will not be accepted. • I understand that the KIDS DRONE ZONE is mandated to report any suspected cases of child abuse or neglect to the appropriate authorities for investigation. • I understand that KIDS DRONE ZONE staff and volunteers are not allowed to babysit or transport children at any time outside the KIDS DRONE ZONE facilities and program. If a violation of this policy is discovered, the KIDS DRONE ZONE will take immediate disciplinary action toward staff and volunteers. Please read carefully Release and Medical Policy All pertinent medical information is up to date and accurate for minor child(ren) I am registering and I understand I must provide all emergency medications (i.e. epi pen, inhaler) before my child can attend any Camp program/s. This does not pertain to a one day activity/field trips. For one day activity/field trip, parents must ensure they have or will administer medications. KDZ will not administer medications for one day/field trip activities. Release: By submitting this payment, I understand there are risks of physical injury in participating in sports and recreational activities or programs. I agree to indemnify and hold Kids Drone Zone, IDO, its parents, subsidiaries, affiliates, officers and employees, harmless from any claim or demand, including reasonable attorney’s fees and costs, made by any third party due to or arising out of client’s use of the Service, the violation of this Agreement, or infringement by client. I hereby release of Kids Drone Zone IDO, its employees, officials and agents from any and all liability or loss or damage to personal property that my child or I may experience in connection with activities sponsored by Kids Drone Zone. I hereby consent to emergency medical procedures deemed advisable for me or for my child in the event I cannot be reached or give consent and either I or my child have sustained an injury. Kids Drone Zone does not provide accident or hospitalization insurance for participants of its programs. All participants are advised to have adequate personal coverage. Please consider participant's own health, experience, and tolerance for risk before participating in any program. I also consent to the use of my or my child’s photo, video, artwork etc. by Kids Drone Zone for flyers, presentations, marketing, etc. RELEASE In consideration of the Kids Drone Zone allowing me and/or my minor child(ren) or ward(s) to attend and/or participate in any programs, events, classes, or other activities at the Kids Drone Zone and/or sponsored by the Kids Drone Zone, I hereby, for myself, my minor child(ren) or ward(s), heirs, and executors, waive, release and forever discharge the Kids Drone Zone and its employees, agents, counselors, teachers, trainers, representatives, successors and assigns, from and against any and all rights and claims for any loss, damage, illness or injuries to person or property sustained as a result of my attendance and/or participation in any such programs, events, classes, and other activities, whether or not such loss, damage or injury results from the negligence of the Kids Drone Zone and its employees, agents, or representatives or from some other cause. My agreement to release the Kids Drone Zone does not include any loss, damage or injury that results from the Kids Drone Zone 's gross negligence or willful, wanton, or reckless misconduct. I further waive any and all rights to inspect or approve the photograph, videotape, television program, motion picture, tape recording or other use of my and/or my minor child(ren)’s or ward(s)’ name(s) and/or likeness(es), including any written article, script, caption or other writing that may accompany such use of my and/or my minor child(ren)’s or ward(s)’ name(s) and/or likeness(es). I hereby, for myself, my minor child(ren) or ward(s), heirs, and executors, waive, release and forever discharge the Kids Drone Zone and its employees, agents, counselors, teachers, trainers, representatives, successors and assigns, from and against any and all liability, claims, losses, costs, expenses or damages for libel, slander, invasion of privacy, conversion, defamation, appropriation of likeness or any other claim based on the use of my and/or my minor child(ren)’s or ward(s)’ name(s) and/or likeness(es) in any such materials. Behavior and Safety Policy: Kids Drone Zone takes the happiness of your children very seriously. All incidents will be handled on a 3 incident system. For the safety and enjoyment of all the children we have the following policy in place: o First, a verbal warning is given. o Second, a written warning is sent home with the child. o As a last resort, if the written warning is not heeded, the child will be suspended from the Program. Scholars who consistently do not follow safety rules will be removed from the program (including use of foul language, hitting/fighting) . If your child should create a disturbance and become unmanageable, or require to have a parent or guardian pick up your child. Future attendance at the program will be reviewed on a case-by-case basis by staff. There will be NO refunds if your child is released from any program. Refund Policy There are no Refunds. There are no refunds for missed classes/sessions. Kids Drone Zone reserves the right to cancel the program if a suitable number of participants fail to enroll or maintain enrollment. RELEASE FORM FOR ADULTS Important: Each volunteer/Adult Participant must read and sign the “Release and Waiver of Liability” before volunteering on a Kids drone Zone. Please complete this form and submit online before you volunteer. Kids Drone Zone (AKA KDZ), a nonprofit corporation organized and existing under the laws of the State of Georgia, USA. Release Kids Drone Zone, their directors, officers, employees, and agents (collectively, “KDZ”) of the following below: I, the Volunteer, desire to work as a volunteer for KDZ and engage in the activities related to being a volunteer/participant for a work project. I hereby freely and voluntarily, without duress, execute this Waiver under the following terms: 1. Waiver and Release. I, the Volunteer/Participant, release and forever discharge and hold harmless KDZ and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer work with KDZ. I understand and acknowledge that this Waiver discharges KDZ from any liability or claim that I, the Volunteer, may have against KDZ with respect to bodily injury, personal injury, illness, death, or property damage that may result from my participation on the KDZ volunteer site. I also understand that KDZ does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage. 2. Insurance. I, the Volunteer, understand that I expressly waive any such claim for compensation or liability on the part of KDZ beyond what may be offered freely by the representative of KDZ in the event of such injury or medical expense. 3. Medical Treatment. I hereby release and forever discharge KDZ from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my time with KDZ. 4. Assumption of the Risk. I understand that my time with KDZ may include activities that may be hazardous to me, including, but not limited to, construction activities, loading and unloading of heavy equipment and materials, and local transportation to and from the work sites. I hereby expressly and specifically assume the risk of injury or harm in these activities and release KDZ from all liability for injury, illness, death, or property damage resulting from the activities of my time with KDZ. 5. Photographic Release. I grant and convey unto KDZ all right, title, and interest in any and all photographic images and video or audio recordings made by KDZ during my work for KDZ, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. 6. Other. I expressly agree that this Waiver is intended to be as broad and inclusive as permitted by the laws of the State of Georgia in the United States of America, and that this Waiver shall be governed by and interpreted in accordance with the laws of the State of Georgia. I agree that in the event that any clause or provision of this Waiver shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable. Volunteer Statement Affirming Understanding I understand that information regarding students, families, staff, and the organization may be confidential in nature and that as a volunteer/Participant for Kids drone Zone. I will… respect the confidential nature of any verbal or written communication I receive regarding students, families, staff, and the organization and keep personal information confidential. I will be discreet in any verbal communication by not discussing students, staff, or families in front of others. immediately report directly to Kids Drone Zone any information disclosed to me concerning a child’s safety. I will make reasonable efforts to assure that each student is protected from harassment or discrimination and I will not harass nor discriminate against any student, staff member or volunteer on the basis of race, color, religion, sex, age, national or ethnic origin, political beliefs, marital status, handicapping condition, sexual orientation, or social and family background. I also understand that relationships developed with children should remain at at the event and that for the protection of both the student, staff and volunteer, volunteers should not be left alone with a child that is out of view of KDZ personnel or another adult volunteer. Volunteers are also reminded that permission to communicate with a student outside of the activity must be granted by the student’s parent/guardian; KDZ cannot and will not grant this permission.
